The templates in the above buttons are all associated with the Illinois Arts Council’s FY25 General Operating Support application.
- Accessibility Self-Assessment Checklist (see page 15 of the Guidelines): The Accessibility Checklist is intended as a self-assessment tool to encourage organizations to continue thinking about how to expand their accessibility. The Checklist is required for an application to be considered complete, but the answers are not evaluated as part of the review process.
Please note the Accessibility Checklist is uploaded under the Applicant Information tab in Salesforce and should be updated on the years your organization is submitting a full application.
- Financial Form (see page 18 of the Guidelines): The Financial Form is a required document for organizations that do not have a mission dedicated to the arts in Illinois. This document is also required for all colleges and universities. Please complete this form with financial information from your most recently completed fiscal year that you have closed book financials.
If your organization does have a mission dedicated to the arts in Illinois, you will be uploading a 990 and do not need this template.
- Board & Staff List (see page 19 of the Guidelines): The Board & Staff Listing is a required supporting document. Use of this template is optional and is intended as a resource. If your organization already has a prepared Board and Staff listing that may be submitted.
