Final Reports for Regional Arts Partners will be filed electronically using the Salesforce System. To learn how to access and use the Salesforce system, refer to the Using Salesforce section of the IAC website.
Deadline
Final Reports must be submitted electronically through the Salesforce system no later than November 15.
Salesforce Form Fields
Financials & Individuals Benefiting
- Total operating income (must include IAC grant amount)
- Total operating expense (must include IAC grant expenditures)
- Total operating in-kind
- IAC grant amount spent
- Brief explanation if the expense was greater than the income
- Total attendance for different in-person activities and events
- Total artists participating for different in-person activities and events
- Total volunteers participating for different in-person activities and events
- Total youth participating for different in-person activities and events
- Total individuals benefitting from different in-person activities and events
National Endowment for the Arts Requirements
Provide estimates of the population that directly benefited from the award during the grant period including by:
- Race / Ethnicity
- Distinct Groups (disability, veteran, poverty, etc.)
- Age
Grant Program Activities Location
Provide number of days of activities and the address(es) of the locations of activities supported by the award during the grant period.
Supporting Documents
Upload each of the following:
1. Narrative:
Attach a one-page narrative, which responds to each of the following questions. Please be sure to restate each question preceding your response.
- Describe the impact of this grant on the organization’s overall mission and activities.
- Describe how the application procedure and review process for your program was actually carried out.
- Identify any specific under-represented populations that have had limited access to the arts and have benefited from this year’s program.
- Explain how your organization evaluates its Regional Arts Partners Program and the project’s grant recipients.
Save the narrative document as a PDF and label as follows Organization name (or a recognizable acronym)_Narrative.pdf.
2. RAP Summary
Complete the Summary of Regional Arts Partners Program (Applications received/awarded) using the template provided. Save the document as a PDF and label as follows: Organization name (or a recognizable acronym)_RAP Summary.pdf.
3. Applicant Listing
Using the Applicant Listing template provided, list information on each applicant to your RAP regranting program for the year you are reporting on. Save the document as an excel file and label as follows Organization name (or a recognizable acronym)_RAP Listing.xls.
4. Financial Report
Attach the provided financial template (pdf or excel) of revenue and expense for the grant period. Refer to the IAC Glossary if you have questions on terms. When the organization’s fiscal year does not coincide with the IAC grant period, use the most recent completed fiscal year (audited when available). Save the document as an excel or pdf file and label as follows Organization name (or a recognizable acronym)_Financials.xls.
5. Documentation
Provide one example of published materials or news releases verifying funded activities and acknowledgment of the IAC (“This program partially supported by a grant from the Illinois Arts Council.”) Acceptable formats for uploaded files are PDF, JPEG, or WORD (as .doc or .rtf files.) When possible, include IAC acknowledgment verification on the first page of your documentation.
Optional Fields
- Provide an optional quote on the impact of the grant.
- Upload a photo to use with the quote.
- Provide a photo credit, if the photo was uploaded.
Authorization / Submission
Certify that the information is correct and submit the report.
