Final Reports for Folk Arts Mentorship will be filed electronically using the Salesforce System. To learn how to access and use the Salesforce system, refer to the Using Salesforce section of the IAC website.
Deadline
Final Reports must be submitted electronically through the Salesforce system no later than November 15.
Salesforce Form Fields
Individuals Benefiting
- Total attendance for different in-person activities and events
- Total artists participating for different in-person activities and events
- Total volunteers participating for different in-person activities and events
- Total youth participating for different in-person activities and events
- Total individuals benefitting from different in-person activities and events
National Endowment for the Arts Requirements
Provide estimates of the population that directly benefited from the award during the grant period including by:
- Race / Ethnicity
- Distinct Groups (disability, veteran, poverty, etc.)
- Age
Project Expenses
- Total income (must include IAC grant amount)
- Total expense (must include IAC grant expenditures)
- Total in-kind
- IAC grant amount spent
- Brief explanation if the expense was greater than the income
Grant Program Activities Location
Provide number of days of activities and the address(es) of the locations of activities supported by the award during the grant period.
Supporting Documents
Upload each of the following:
- Mentor Narrative: 1-page narrative answering these questions:
- Were there any significant changes to the original application’s work plan? If so, describe these changes.
- Describe the progress the Apprentice achieved during the apprenticeship period.
- Describe the public presentation that you and the Apprentice conducted. Specify how you publicized it and who attended.
- Are there any plans to continue working with the Apprentice? If yes, describe your plans. If not, explain why you will not continue.
- Please share additional aspects of this mentorship that were meaningful to you. (E.g. history, traditional tales, customs, etc.)
- Apprentice Narrative: 1-page narrative answering these questions:
- Did the work plan support your learning the art form?
- Describe the progress you achieved during the apprenticeship period.
- Do you wish to continue working with the Mentor? If yes, describe your plans. If not, explain why you will not continue.
- Please share additional aspects of this mentorship that were meaningful to you. (E.g. history, traditional tales, customs, etc.)
- Attach the provided financial template (pdf or excel) of revenue and expense for the grant period. Refer to the IAC Glossary if you have questions on terms. When the organization’s fiscal year does not coincide with the IAC grant period, use the most recent completed fiscal year (audited when available).
- Mentor: Provide documentation of the public presentation.
- Mentor: Provide one example of published materials or news releases verifying funded activities and acknowledgment of the IAC (“This program partially supported by a grant from the Illinois Arts Council.”) Acceptable formats for uploaded files are PDF, JPEG, or WORD (as .doc or .rtf files.) When possible, include IAC acknowledgment verification on the first page of your documentation.
Optional Fields
- Provide an optional quote on the impact of the grant.
- Upload a photo to use with the quote.
- Provide a photo credit, if the photo was uploaded.
Authorization / Submission
Certify that the information is correct and submit the report.
