The application narrative will be rated against the following five Review Criteria: (refer to the Evaluation Criteria section for more details)

  • Pursuit of Excellence
  • Organizational Capability
  • Community Involvement
  • Serving Illinois Artists
  • Evidence of Community Support

Use each of the above criterion as a heading in the narrative while addressing the program specific topics listed below. Failure to do so will jeopardize your eligibility.

To assist applicants, the corresponding criterion is/criteria are provided in parentheses after each standard. Please provide demographic characteristics of the community being served and discuss how your organization meets each of the standards of performance listed below:

  1. governance by a non-self-perpetuating board which reflects the needs of the organization and community it serves; (meets Criterion II)
  2. evidence of ongoing community cultural assessment and planning that involves the diverse population of the community (meets Criterion III)
  3. general promotion and encouragement of the public to understand excellence in the diverse art forms represented in the region (meets Criterion I)
  4. continuity of professional (paid or volunteer) management and year-round operations (meets Criteria II & III)
  5. services to a defined geographic area (meets Criteria III & IV)
  6. provision of at least three of the following programs and services in a variety of arts disciplines:
  • regranting funds to artists and organizations
  • producing or presenting programs not otherwise offered within the region (e.g., festivals, public art, exhibitions, concerts, workshops, etc.)
  • providing technical assistance to artists and arts groups
  • coordinating arts-in-education programs for schools and the community
  • functioning as a cultural advocate in the community
  • fund-raising for the benefit of artists and arts groups that provide programming in the service area
  • providing or managing facilities for the creation or presentation of the arts (describe programs and services primarily under Criteria I, III, IV and V)

organizational history

One-page organizational history which relates to the program area of the applicant

Key Personnel

One-page containing paragraph descriptions of each key personnel (paid and/or volunteer) which includes job title, workload (e.g. full-time or part-time) and brief biography describing their training

objectives and measurement

One-page description of orgranization's or project's objectives and intended measurement of these objectives for FY2004.

Fifteen/five copies of a one-page budget outline detailing administrative expenses, artistic fees, admissions/earned income and corporate/ foundation and other private support.

program specific attachment    15 copies   5 copies  what's this?

Fifteen/five copies of the organization’s current strategic plan must be submitted. If this plan is under review, the most recent draft may be submitted with a paragraph describing a plan for completion. If no strategic plan exists, provide fifteen copies of a description of the organization’s long-term objectives with timeline for completion and intended measurement of these objectives.

Fifteen/five copies of the organization’s proposed schedule of events and services must be submitted. Calendar should begin with September 2004 and end August 2005.

budget explanation

One page explaining any increase or decrease on any budget line on the "Summary of Total Public Arts Programming Budget" page which exceeds 20% between fiscal years

Current proof of not-for-profit status

One copy of the organization's current fiscal year's Illinois not-for-profit status. What is accepted as proof of not-for-profit status?

audited financial statements

For organizations with cash income of $150,000 and above, one copy of audited financial statements

current list of board of directors

Two copies of the current list of board of directors

support materials

Two sets of up to five samples of recent publications, promotional materials and/or programs/reviews must be submitted. (If applicant currently receives IAC support, include samples which acknowledge that support.)

job description

If requesting funds for a position, two copies of one-page job description

back
