The application narrative will be rated against the following five Review Criteria: (refer to the Evaluation Criteria section for more details)

  • Serving Illinois Artists
  • Evidence of Community Support

Use each of the above criterion as a heading in the narrative while addressing the program specific topics listed below. Failure to do so will jeopardize your eligibility.

project budget Detail

Only if applying for project support. Included in the application download entitled, "Project Budget Detail."

organizational history

One-page organizational history which relates to the program area of the applicant

Key Personnel

One-page containing paragraph descriptions of each key personnel (paid and/or volunteer) which includes job title, workload (e.g. full-time or part-time) and brief biography describing their training

objectives and measurement

One-page description of orgranization's or project's objectives and intended measurement of these objectives for FY2004.

Fifteen/five copies of a one-page narrative explaining any amounts listed under contractual services and other revenue. Explain any circumstances involving a surplus or deficit and plans on resolving a deficit if applicable. Please list names of anticipated corporate or foundation support. If discounts are offered, please describe, i.e. seniors, children, etc. Please identify sources of in-kind contributions.

The following must be submitted:

  • Fifteen/five copies of a one-page listing of the current season’s events
  • Fifteen/five copies of a one-page listing of the proposed season’s events

Include performance location, number of performances, ticket prices, attendance statistics, breakdown of paid performers (indicate whether they are professionals or students), if work by an Illinois choreographer is used indicate the name of the choreographer. Also include any outreach activities in schools, senior facilities, etc.

Please Note: Dance companies should indicate if the events listed are self-produced (SP), contracted (C), or sponsored (S). If discounts are offered, please describe, i.e. older adults, children.

budget explanation

One page explaining any increase or decrease on any budget line on the "Summary of Total Public Arts Programming Budget" page which exceeds 20% between fiscal years

Current proof of not-for-profit status

One copy of the organization's current fiscal year's Illinois not-for-profit status. What is accepted as proof of not-for-profit status?

audited financial statements

For organizations with cash income of $150,000 and above, one copy of audited financial statements

current list of board of directors

Two copies of the current list of board of directors

support materials

Two sets of up to five samples of recent publications, promotional materials and/or program reviews must be submitted. (If applicant currently receives IAC support, include samples which acknowledge that support.)

One copy of VHS videotape must be submitted. Tape must be of performance produced within the last two years, should be at least of ten-minutes duration and cued primarily to the work of the company members and not guest artists. Excerpts of various pieces are preferable to one full-length work. Tape should be within the framework of what is possible in a live performance. Special effects, dissolves, superimpositions, etc., are highly discouraged. Label tape with applicant’s name, title of work, date premiered, sound credits and total length of work. Please preview tape for sound and cueing.
