Include performance location, number of performances, ticket prices, attendance statistics, breakdown of paid performers (indicate whether they are professionals or students), if work by an Illinois choreographer is used indicate the name of the choreographer. Also include any outreach activities in schools, senior facilities, etc.

Please Note: Dance companies should indicate if the events listed are self-produced (SP), contracted (C), or sponsored (S). If discounts are offered, please describe, i.e. older adults, children.

One copy of VHS videotape must be submitted. Tape must be of performance produced within the last two years, should be at least of ten-minutes duration and cued primarily to the work of the company members and not guest artists. Excerpts of various pieces are preferable to one full-length work. Tape should be within the framework of what is possible in a live performance. Special effects, dissolves, superimpositions, etc., are highly discouraged. Label tape with applicant’s name, title of work, date premiered, sound credits and total length of work. Please preview tape for sound and cueing.

As part of the narrative, describe the ethnic and folk arts activities which the organizations will conduct during the grant period. Include information regarding not only who, how, where, and when, but also why these events will take place. Describe what is traditional in the proposed arts components and explain their significance with the community and if relevant, to the broader audience. Describe how traditional arts activities assist in fulfilling the organization’s mission statement. If appropriate, describe how these activities strengthen the understanding and preservation of the applicants and Illinois’ cultural heritage.

An organization applying for single event funding, including festivals should indicate how the event contributes to expanded on-going arts activities in the community. This should be demonstrated through the involvement of artists and other individuals whose heritage reflects the communities or ethnic groups being represented at the festival/event including planning, design and production activities. Applications for single events should also list specific names of artists and artistic groups who will be included in the event.

If applicant plans to apply to the Artstour Program for partial funding for any of the listed performers/exhibits, indicate each with an (AT).

In order to document artistic quality and tradionality of arts activities, applicants are strongly encouraged to submit one of the following categories of audio visual support materials:

NOTE: Include a one-page audiovisual documentation sheet which includes names of artists/artistic groups, and if relevant: description and date of performance (s), programs, and/or exhibits names and description of musical selection and/or dances. Label all materials with applicant’s name and return address.

One copy of VHS videotape
Tape should be of at least tenminutes duration and cued to the work which best represents the applicant’s artistic quality. Short segments which show the range of an applicant’s artistic programs are encouraged.

Or Cassette audiotape or Compact Disc
Tape/CD should be of a least ten-minute duration and cued to work which best represents the applicant’s artistic quality. Label CD with track number of each selection.

Or Slides
Maximum of nine slides, which are suitable for carousel slide projector. Label each slide with the artist’s name and type of work. Place a red dot on the bottom left-hand corner of the slide. Glass slides cannot be accepted.

These schedules should include names of readers, their genre, event dates and attendance figures (actual and anticipated, paid and complimentary).

  6. provision of at least three of the following programs and services in a variety of arts disciplines:
  • regranting funds to artists and organizations
  • producing or presenting programs not otherwise offered within the region (e.g., festivals, public art, exhibitions, concerts, workshops, etc.)
  • providing technical assistance to artists and arts groups
  • coordinating arts-in-education programs for schools and the community
  • functioning as a cultural advocate in the community
  • fund-raising for the benefit of artists and arts groups that provide programming in the service area

One audiotape or compact disc or 1/2” videotape of completed work or work-in-progress must be submitted if applicant is requesting funds for production. (Promotional tapes will not be accepted.) Tape should be at least of five minutes duration, related to the program for which funds are being requested, demonstrate production values and emphasize creative use of the medium. Panelists will listen or view a minimum of five minutes of the tape and no more than ten minutes. Radio applicant’s tape should feature work actually broadcast (with no promotional voice over) and should be a sampler of strongest work if applicant broadcasts diverse range of programs. Label tape and provide a one-page description of its contents.

Explain any circumstances involving a surplus or deficit and plans on resolving a deficit if applicable. Please list names of anticipated corporate or foundation support. If discounts are offered, please describe, i.e. seniors, children, etc. Please identify sources of in-kind contributions.

Current proof of not-for-profit status

One copy of the organization's current fiscal year's Illinois not-for-profit status. What is accepted as proof of not-for-profit status?

For organizations with cash income of $150,000 and above, one copy of audited financial statements

Two copies of the current list of board of directors

support materials

Two sets of up to five samples of recent publications, promotional materials and/or program reviews must be submitted. (If applicant currently receives IAC support, include samples which acknowledge that support.)

One copy of an audiotape or compact disc for vocal ensembles, orchestras, chamber music, jazz ensembles, contemporary and early music groups, concerts and wind band organizations. Opera companies must submit one copy of VHS videotape. Tape must be of a live performance conducted within the last two years, should be at least ten minutes in duration and cued to the work of company members and not guest artists. Label tape with applicant’s name, title of work, composers name and date of live recorded performance. Please preview tape for sound and cueing.
